FEES: $600. Includes 4 days of workshops (3 hours each) and 3 afternoon 1-hour discussions.

EARLY REGISTRATION: $50 discount on fee balance for participants who register by August 15, 2012.

REGISTRATION: $200 registration fee reserves your space and must be included with Registration Form.

Refundable until December 1, 2012.

BALANCE DUE: Deadline for payment of balance is November 15, 2012.

Mail your completed and signed Registration Form and registration fee to: USPiM, P.O. Box 4150, Grand Central Station, New York, NY 10163. You will automatically be entered in our Merida Fellowship Award if received by August 31, 2012.

*Returning participants receive a $75 discount on tuition.

*Cancellations after December 1, 2012 will result in no refund.
